Just for kicks, remove your Profile picture... Then, try to add your Avatar again. This would be like doing what Mike said... by renaming.
If this works, there was a name conflict whereby the same file could not be used for both Profile & Avatar...
Just a shot in the dark... Try it & let us know what happened.
