Default Router backout danger

I have a PorterCable 690 router. It mounts below the table (also PorterCable) in a base that allows height adjustment by rotation. There is a large wing bolt to secure the router after adjusting the height. Despite attempts to tighten it firmly, I am having consistent problems with it backing out. Sometimes the result is that the height of the cut is lower, but once it fell out of the table and destroyed a bit. I was lucky that was all. Is this common, or is there somehting wrong with my mount?
