Default Slipping bits

Hello All,
We have a Sabre 408 and haven't had many issues in the past. However, over the past month, we have had 3 cases where the bit slipped down breaking the bit and damaging the table. Any suggestions??

Maybe the collet is worn & not holding anymore.

Have you had a change of operator? Are you using a spindle lock or two wrenches? Which motor the 7HP or the PorterCable 3?

Are you using small bits with collets or sleeves? Did someone do any maintenance on the machine? Look for improperly assembled subsets, i.e. wrong bearing slightly offset just enough to throw out the tracking of the armature but not enough to disturb other things except the bit. Is there a wobble in the end of the armature near the collet? These gantry stepper motors can be some powerful and when the bit hits something it can get a pretty good smack.
