I know it sounds strange but, for a particular need I have I think it could be useful to somehow carve a groove in EPS foam.

Is it possible/adviceable?.

Yes it is possible but without more info I don't know if it is advisable.

If the question is how to cut the groove then do a youtube search on hot wire cutter.

eps = encapsulated polystyrene? Then I agree with Al that a hot wire cutter is the way to go. I've done this numerous times and with a little practice you can get super clean results. The trick is to build a bow to hold the wire tight and cut at the right rate. You can also saw it with a hand saw or even just a hacksaw blade if your shape is irregular.

Routing it will surely make a huge mess.

Yeah, saw some videos and indeed it seems to be a better choice to just build a hot wire cutter.
