Squaring up my work piece, driving me nuts !

I am cutting 12" x12" square pieces .75 thick. I zero in the center of the piece as close to the center of my waste board towards the front. The top and bottom seem to be OK, but the left and right are off, and if I cut a border around the scripture it really shows left / right. I have moved it back and forth as per it seems to be off, but still can't get it exactly right.. side to side that is .
I know it is what we used to call operator error, but until one of you wonderful folks tell me what I am or am not doing I am scraping wood. I could deal with it if it were but a few thousands off, but .375 - .50" is way off.

Sorry not the moment , I have a 12" x 2" piece of wood I am butting it up too, as center of my waste board as I can get it screwed to the waste board, ??? putting the 12" x 12" piece flush on both sides clamping it to it
Hey Mr.David,
I came home from work thinking about what I needed to do, it took a few tries but I think I have figured it out. The piece I was using to clamp to, a stop block needed to be half the distance differences on X and Y. Making one a little larger, and moving my stop over to the right just a tad has it looking great,, caliphers instead of a tape measure much better as well.
