Default Hinge Recommendation

I'm building a presentation case for a friend that will have an item of sentimental value. The dimensions are 36" L x 11" W x 4"-5" H of 3/4" Mahogany or Cherry. What type of hinge would be appropriate for this project.

Additionally, would it be better to cut the lid from the box or build the lid separately.

Cut the lid from the box...
dull/antique piano....
four ornate knife hinges

You might also want to consider lid stays as well as the hinges. That's gonna hurt if (when?) that lid comes down on someone's hand!

Maybe a quadrant hinge? I looked at them and din't see a load rating for the ones I looked at, may be a problem as this is not that small of a lid.
